Instructions for Completing Nomination Form Below
2025 Nominations are now open.
- Nomination form and letters of recommendations must be completed and submitted digitally by 12:00 PM CT on Friday, November 1, 2024.
- Mail-in and email applications will NOT be accepted. The PDF version of the application provided is solely for reference only. PDF Link
- No other attachments, beside letters of recommendations, will be accepted as part of the nomination form.
- Letters of recommendation are limited to three (3) total. Each letter is limited to one (1) page and must be written in a size 12 font to ensure legibility. Letters can be uploaded as PDFs at the end of the application.
- The selected EOY will be notified mid-December 2024.
