2024 Nominations are now open.

Nomination form and letters of recommendations must be completed and submitted digitally by 5:00 PM CT on November 1, 2023.
Mail-in and email applications will NOT be accepted. The PDF version of the application provided is solely for reference only. PDF Link
No other attachments, beside letters of recommendations, will be accepted as part of the nomination form.
Letters of recommendation are limited to three (3) total. Each letter is limited to one (1) page and must be written in a size 12 font to ensure legibility. Letters can be uploaded as PDFs at the end of the application.
The selected EOY will be notified mid-December 2023.
